This cyclical process emphasises that risk control is never static; instead, it evolves continuously through deliberate planning, implementation, monitoring, and review activities. Central to this cycle is communication, which acts as the mechanism through which data flows, decisions are made, responsibilities are understood, and learning is captured across the organisation.
The effectiveness of risk control measures is directly linked to how well communication enables hazard identification, distribution of safe systems of work, reporting of incidents and near misses, and escalation of findings from both proactive and reactive monitoring. When communication systems work effectively, the organisation is able to identify early warning signs, intervene before accidents occur, and update controls in response to emerging information. Conversely, when communication fails, even well-designed risk controls can break down, leading to preventable incidents, regulatory breaches, and cultural deterioration.
This Knowledge Providing Task examines how the continuous risk control loop interacts with structured communication systems and how both proactive and reactive monitoring activities feed into the PDCA cycle. The task invites the learner to develop a detailed conceptual model that illustrates the interdependence between risk control, monitoring activities, and organisational communication pathways. In doing so, the learner must demonstrate a deep understanding of how information flows across an organisation, how evidence from incidents triggers mandatory review processes, and how communication improvements enhance regulatory compliance, stakeholder engagement, and overall safety performance.
1. Task Purpose (What the Learner Must Demonstrate)
This task requires you to develop a detailed, professionally structured, and deeply analytical model that illustrates the continuous cycle of risk control and communication, showing how:
- Formal risk control planning and implementation (Plan–Do) depend on structured, reliable communication systems.
- Proactive and reactive monitoring activities (Check) generate intelligence that must be formally communicated, analysed, and escalated.
- Incident data and reactive indicators drive mandatory reviews of both proactive monitoring systems and the original control measures.
- The organisation continually updates and realigns its Health & Safety communication systems (Act) to maintain legal compliance, stakeholder engagement, and safety culture maturity.
This task must reflect the UK regulatory expectations under:
- Health and Safety at Work etc. Act 1974 (HSWA)
- Management of Health and Safety at Work Regulations 1999 (MHSWR)
- Reporting of Injuries, Diseases and Dangerous Occurrences Regulations 2013 (RIDDOR)

2. PART 1 — Develop a Detailed Flow Model of the Continuous Risk Control Loop
Create a conceptual diagram or a detailed, step-by-step process map showing how the organisation’s communication systems support the entire Plan–Do–Check–Act (PDCA) cycle. Your model must include multiple layers of communication, governance, and data flow.
Mandatory Components of Your Model
PLAN Phase – Risk Control Planning Through Structured Communication
Your model must show how the organisation:
- Communicates hazard information collected from inspections, consultations, near-miss reporting, and risk assessments.
- Uses structured communication channels (toolbox talks, formal meetings, safety alerts, digital dashboards) to ensure all stakeholders contribute to risk planning.
- Integrates frontline feedback into the risk assessment development process.
- Ensures all information is accessible to employees, temporary workers, contractors, and senior management, in accordance with HSWA s.2 and MHSWR Reg. 10.
Include: - Upward communication (workers → supervisors → H&S team → directors).
- Horizontal communication (between departments, contractors, or project teams).
- Downward communication (policy instructions, safe systems of work).
DO Phase – Implementing Controls and Communicating Responsibilities
Show how the implementation of risk controls relies on:
- Clear dissemination of safe systems of work (SSoWs).
- Task-specific briefings (e.g., toolbox talks, method statements, RAMS communications).
- Verification of understanding using questioning, sign-off sheets, competency checks, or digital acknowledgements.
- Real-time communication channels such as radio networks, digital permit-towork systems, and on-site noticeboards.
- Accessible documentation for non-English speaking workers or workers with additional needs.

3. PART 2 — Integrate Proactive and Reactive Monitoring Into the Feedback Loop (CHECK)

Examples include:

- Behavioural safety observations

- Monitoring of SSoW use
Show how this proactive data is: - Collected, communicated, and escalated
- Analysed for trends or weak signals
- Shared with management and workers via dashboards, reports, or formal meetings
Explain how HSG65 emphasises that proactive indicators reveal the health of the management system before incidents occur.
B. Reactive Monitoring (Incident-Driven Feedback)
Examples include:
- Accidents
- Near misses
- Dangerous occurrences (RIDDOR reportable)
- Equipment failures
- Enforcement notices
- Complaints from workers or reps
- Non-conformities during audits
Your model must show:
- Immediate communication channels for reporting (verbal, radio, digital apps, emergency lines).
- Formal investigation reporting flows from worker → supervisor → H&S team → top management.
- How RIDDOR creates mandatory communication to the enforcing authority.
- How incident findings are communicated organisation-wide through safety alerts, meetings, and policy updates.
4. PART 3 — Explain How Reactive Data Forces Mandatory Review of Proactive Metrics and Risk Controls
You must provide a detailed narrative explanation covering:
How Incident and Near-Miss Data Challenges Proactive Monitoring
Describe how a reactive event triggers:
- Reassessment of inspection frequencies
- Review of behavioural safety trends
- Evaluation of training effectiveness
- Revalidation of communication pathways
- Checks for “communication failures” as root causes (e.g., unclear instructions, outdated SSoW, language gaps)
Explain how failures identified during an incident investigation must inform future proactive monitoring priorities under MHSWR Reg. 3 & Reg. 5 (arrangements for planning, organisation, control, monitoring and review).
How Reactive Data Leads to Mandatory Review of Original Control Measures
Provide detailed reasoning on:
- Why a risk control measure is considered not effective if an incident occurred despite it.
- How incident trends or repeated near misses indicate systemic failures.
- How control measures must be modified, redesigned, or replaced (e.g., engineering before administrative controls).
- How modifications must be communicated clearly and promptly to all affected parties.
- How leadership must be informed to update resource allocation.
